Children’s Community Nursing Team (CCNT)
We provide holistic family centered nursing care, support and advice for children and young people with an acute, chronic or complex healthcare need. We offer nursing care for children from birth to 16 years. Although we can continue to provide care to young people aged up to 19 years who remain under the care of a consultant paediatrician.

SEND (Special Educational Needs & Disabilities) and CCNT involvement with EHC’s (Education, Health and Care Plans)
Nurses may be requested to contribute to Education, Health and Care (EHC) Plans and annual reviews for children and young people with additional needs. Once a request is received for contribution to an EHC Plan, the Nurse will work within legal time-frames to collate this information in collaboration with the parents. Our aim is to work in partnership with parents/ carers to provide holistic family support so that children are able to maintain optimum health, remain at home and access education.
Our Nurses provide clinical training for schools and nurseries so that children who require additional support can attend school. We work particularly closely with Specialist Educational establishments within the Wigan Borough.
We provide medical devices and equipment as well as consumables to the Children of the Wigan Borough.
Who might be referred to us?
- Children with a chronic/long term health care needs that require nursing care, ongoing support and advice on health conditions.
- Children and young people who are reliant on medical technology, for example, naso-gastric feeding tube, gastrostomy, tracheostomy and ventilation support
- Children and Young People with life limiting or life threatening conditions who require nursing care or support.
- Children and young people who require a package of care to support their care at home- The CCNT liaise with, and support, the local commissioning board, Children’s Continuing Care Matron and care agencies to ensure that the child’s/young person’s needs are identified and addressed.
- Facilitate preferred place of care for the end of life for a child/young person and their family.
The Children's Community Nursing Team provide training by experienced, registered Children’s Community Nurses. Training can be provided to parents/ carers and education staff so that everyone looking after your child understands their needs and how best to care for them.
Examples of training that can be delivered include:
- Gastrostomy care
- Administration of oxygen
- Tracheostomy care & airway management
- Oral and nasal suction
- Advice and support on epilepsy and seizure management.
The Children's Community Nursing Team are a recognised placement for student nurses completing nursing degrees from regional universities, this includes sign-off mentors for students at the end of their formal nurse training.
The Children’s Community Nursing Team is made up of different workstreams:
Acute: Care closer to home for acutely unwell children, facilitating early discharge and preventing unnecessary re admission to hospital.
Specialist Lead Nurses: Epilepsy and Cystic fibrosis: Manage a caseload of children to support them and their families.
Complex: Care for children with life limiting or life-threatening illnesses who require a nursing intervention. This includes End of Life Care in the home supported by Derian House Children’s Hospice
Referrals to the team for both acute and complex needs referrals should be sent electronically to the email addresses on the referral form.
