- How to Access the Service
A request for podiatry treatment should be made by completing our Podiatry referral form. Or by phoning 0300 707 7700 with a list of your medication and the referral can be completed by the admin team for you.
These can be completed on behalf of the patient by other health care professionals.
The form asks for details about a patient’s general health and their medication.
These are available at GP surgeries or Clinics/Health Centres.
It is important to fill in all parts of the podiatry referral form so that the application can be properly processed.
Application forms should be posted to:Podiatry Administration,

- What Happens Next?
Your application will be triaged against the service access criteria.
Assessment appointments will only be sent to those people who have met the access criteria. Treating minor foot problems and nail care in the healthy individual is no longer the role of the NHS Podiatrist.
At the initial appointment your medical and podiatric needs will be assessed and a decision taken as to future ongoing podiatry provision. This will then be reviewed on a regular basis.
