Subject Access Requests under the Data Protection Act 2018
If you want to ask us for information which we may hold about you then this will be dealt with under the Data Protection Act 2018. This includes health records, employee records and any other record we may hold about you such as complaints / incident reports. If you would like to see your health records or would like copies, you will need to send a written request or complete an application form which are available below.
Please note that proof of identity is needed to process an Access to Health Record Request. This can be a copy of a formal document or bill, with your name and address on it, or a copy of your driving licence or passport. Please ensure that you provide any supporting documents to the Access to Health Records team using the below contact details:

PLEASE NOTE: From 8th July 2019, our default method of sending records will be via a download service. If you wish to still receive your records via post, please specify on the request form.
We have one calendar month to provide the information you ask for. If your request is deemed to be complex this is extended by a further two months. If you are unhappy with any aspect of the way in which we deal with your subject access request, please contact the Information Governance Department. If you are still unhappy with the response you receive, you can contact the Information Commissioner's Office (ICO). The ICO regulate the Data Protection Act 1998. Their contact details are:
